Declaration of consent to the processing of personal data
I hereby give my consent to the processing of my personal data by Super Krak S.A. with its registered office in Kraków (post-code 31-231, ul. Bociana 6, phone: 12 61 64 108 (106) e-mail: sekretariat@superkrak.com.pl,
as Personal Data Controller (hereinafter: PDC),
1. Personal data will be processed on the basis of consent only for the duration of the granted consent.
2. Your personal data may be disclosed by the PDC to the following parties:
- PDC employees/associates authorised to process them at the request of the PDC in order to achieve the objective indicated in the consent,
- the entities to which the PDC has entrusted the processing of personal data, including technical and organisational service providers to the extent necessary for the PDC to properly provide its services, in particular, IT service providers, in compliance with the data security guarantees required by law,
- institutions operating and authorised by the provisions of the law.
3. The consent to the processing of personal data is voluntary.
4. You have right to access your data and to correct them if they are incorrect.
5. At any time you have right to request to delete the data and limit its processing, as well as to transfer the data, to object to the processing of the data and to withdraw at any time your consent to the processing of the data.
6. Withdrawal of consent shall be effected by sending an e-mail to the e-mail address ODWOLANIE@SUPERKRAK.COM.PL indicated above, or by post to the address indicated above, or by filling in the form below and providing it to the PDC. Withdrawal of consent will result in the immediate cessation of the processing of your personal data and their deletion by the PDC.
The withdrawal of consent shall not affect the lawfulness of processing before its withdrawal.
7. The PDC does not intend to transfer your personal data to recipients located outside the European Economic Area, with the exception of situations beyond the control of the PDC, namely by communicating with it through entities processing data in this way (e.g. entities selected by you in the field of ICT services).
8. You will not be subject to automated decisions (without human intervention) and your personal data will not be profiled unless you give your consent in this regard to the PDC in order to build up an adequate offer of services for you, including a promotion package.
9. You have right to lodge a complaint with the President of the Personal Data Protection Office (or another relevant supervisory authority) when you consider that the processing of personal data by the PDC violates the provisions of applicable law, in particular Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter: “GDPR”).
The PDC shall ensure personal data protection measures in accordance with the legislation in force, in particular in accordance with the GDPR.
